Traduction d'un e-mail : Anglais vers Français

j'aurais besoin d'aide pour traduire une partie d'un e-mail où l'on me demande des vérifications pour passer une commande à l'étranger.

Voici le passage en question :

Acceptable verification information includes one of the following: (1) A copy of the credit card used to order, with only the last 4digits showing AND a copy of your government issued ID (Credit Cardorders only)(2) A copy of a statement from a service provider or financialinstitution, that shows the billing address submitted during checkout(Credit Card or PayPal orders).
